OOSH ENROLMENTS

All parents must complete a 2010 enrolment form. Please note that 2009 enrolments do not carry over into 2010. Therefore whether or not you are currently using the OOSH program, without completing a 2010 OOSH enrolment and returning it before the end of 2009 your child/children will not be able to use the OOSH program in 2010. 

The 2010 package includes:

• OOSH Enrolment form for 2010
• OOSH Authorisation form for 2010
• OOSH Activities Timetable for 2010
• OOSH Activity Enrolment for 2010
• OOSH 2010 Survey

Placements for activities are limited and go on a first come first served basis only! Activities are NOT automatically carried over from 2009 to 2010. We cannot confirm a place in OOSH for any families who do not return their OOSH enrolment before the end of 2009.

*All packages must be returned before the end of the school term. Please do not leave it until next year to return. Even if you are unsure of the days you require please return your enrolment as you can then just phone the program during Term 1 to confirm your days.
